Visual: Empty Hands - phantom tool

As indicated in the screenshot, I have no item equipped and the character is showing as if I have a tool equipped.

Do you remember how you got stuck in the animation? did you equip and unequip the tools fast, or did you equip via hotbar and or inventory etc.
